A Message About Insurance and Your Care

While we work closely with insurance providers to help you get the most from your benefits, it’s important to understand that we don’t work for the insurance companies — we work for you.

Our priority is to recommend treatments that are best suited to your individual needs, not dictated by insurance coverage. Most dental plans offer great benefits, and we’ll do everything we can to help you maximize those benefits. However, our care decisions are always based on what’s right for your health, not what your plan happens to cover.

We accept most PPO plans, including the following:

• Anthem

• Ameritas

• Blue Cross Blue Shield (most PPO plans; some exclusions may apply)

• Beam

• Cigna

• Delta Dental of Arizona

• Envolve

• GEHA

• Humana

• MetLife

• United Concordia

• United Healthcare

We also accept Medicare Advantage plans from:

• Aetna

• Cigna

• Delta Dental

• Devoted

• Humana

• United Healthcare

Important Updates

• As of April 1, 2025, we are out of network with Guardian Insurance.

• As of May 1, 2025, we are out of network with Aetna.

Even if your plan is out of network, we’re happy to submit claims on your behalf and will gladly perform a courtesy benefits check when you schedule your appointment.

Please reach out to your insurance carrier to confirm what your plan specifically covers.

Financing with Care Credit

If you are interested in financing your dental treatment instead of paying upfront, we accept third-party financing through CareCredit. CareCredit provides patients with many choices for financing. An application for CareCredit can be found at their website, or you can pick up an application from our office during your visit.

No Insurance? We’ve Got You Covered.

At Union Hills Family Dental Care, we believe everyone deserves access to quality dental care — with or without insurance.

That’s why we offer the Direct Care Dental Plan, an affordable in-house membership plan designed for patients without dental insurance.

Your membership includes:

• Free routine exam and X-rays

• Two cleanings per year
  (Applies to routine prophylaxis. Periodontal cleanings not included — but special pricing is available.)

• Up to 20% off restorative services

We also offer low-interest, flexible financing options to help make any additional care more manageable.
